Admission Criteria
  1. Mental Health Diagnosis
  2. Physical Diagnosis (if applicable)
  3. Stable
  4. No history of verbal or physical violence
  5. Submit diagnosis/history/physical and medication records
  6. All medications/over the counter medications must be listed on application
  7. Submit Medicaid/Medicare/Insurance Information
  8. Current TB (tuberculosis) test (within one year)
  9. Current Physical (within one year)
  10. No Special Diets
  11. Ambulatory
  12. Adequate funding in place for Residential Care cost
  13. 21 years or older
  14. No history of Pyromania
  15. Current physician orders (Mental Health and Health)
  16. No Registered Sex Offenders
  17. Two Week Supply of all medications
***** Diagnosis/History/and Physical must be received
before acceptance into program. Once client is accepted,
documentation of all benefits, physical, and TB test must be
presented prior to admission.

ask that you make an appointment. Once we receive the
information from the provider and the general information form
(two pages) from you, it will be reviewed to see if admission
criteria is met.
